UNLIMITED TWELVE-MONTH MEMBERSHIP CONTRACT
I hereby authorize B-Risque Fitness to initiate debits on the first of each month to the Bank indicated below, hereinafter called the Bank, and authorize said Bank, to debit $28.00 a month for twelve (12) months to my account. 
I understand that this authorization for automatic withdrawal will remain continuous throughout my twelve month unlimited monthly contract with B-Risque Fitness for $28.00 per month. I understand by my signature below that I am issued a discounted rate of $28.00 per month with a twelve-month contract and this unlimited contract. Should I cancel my contract, a $100.00 fee is due to B-Risque Fitness with a written cancellation notice mailed to B-Risque Fitness 8816 University East Drive Suite D, Charlotte, NC 28213. 
Should I default on any payment obligation as called for in this agreement, B-Risque Fitness will have the right to declare the entire remaining balance due and payable and I agree to pay allowable and all costs of collections. A default occurs when any payment due under this agreement is past due. Should any monthly payment become more than 10 days past due you will be charged a late fee of $10.00. I also understand that an additional service fee of ($25.00 NSF FEE & $5.00 for credit/debit) will be accessed for any check, draft or credit card returned/declined for non-payment. B-Risque Fitness reserves the right to draft member’s account for all amounts owed including any late fees and service charges. This contract will terminate at the end of the twelve months. I understand that I must update B-Risque Fitness with any new information or any bank/credit/debit account changes. The privilege of making payments under this contract may be revoked by B-Risque Fitness if any item is not paid upon presentation. If this preauthorized payment arrangement is revoked for any reason this does not release me from my obligation to pay. By signing this contract I understand that I have entered into an installment promissory note and failure to use the membership does not relieve me from my liability for payment. My membership is absolutely non-transferable, non-assignable, non-refundable and non-cancel-able except as provided in this contract. 
I understand by signing this document that this contract is a binding agreement, non-refundable, non-transferable. B-Risque Fitness will automatically redraft my fee if they are unable to debit my account because of account changes or insufficient funds. B-Risque Fitness reserves the right to cancel an unlimited membership if an account cannot be debited.

Notice to Buyer: Do Not Sign This Contract Before You Read It. Buyer has the right to cancel this contract at anytime prior to midnight of the third business day after the date of this contract. To cancel you must notify B-Risque Fitness at 8816 University East Drive Suite D, Charlotte, NC 28213 in writing no later than midnight of the third business day after the contract date. Business days are all days other than Saturdays, Sundays and Holidays.

I understand that I am purchasing a ten (10) class card to B-Risque Fitness. I can attend any ten classes of my choice except for workshops. I also understand that this class card will expire within 90 days from purchase. I must use all classes within the 90 days.

All merchandise is refundable within 5 business days if unused.

I understand that I must pay a $100.00 deposit to reserve my GNO.  I also understand that 3 days prior to the GNO, I must pay my remaining balance of $149.00 for up to 10 guests. I must reschedule or cancel 3 days prior to the date of the event.
